Mounjaro was supposed to help people manage diabetes and lose weight. For many users, it did exactly that. But for thousands of others, the drug caused serious medical complications that changed their lives for the worse.

As of 2026, over 3,500 lawsuits have been filed against Eli Lilly, claiming the company failed to warn patients about the true risks of Mounjaro. Three injuries appear more than any others in these cases.

1. Gastroparesis (Stomach Paralysis)

Gastroparesis is by far the most common injury cited in Mounjaro lawsuits. This condition occurs when the stomach muscles stop working properly, leaving food trapped in the stomach for hours or even days instead of moving through the digestive system normally.

Mounjaro works partly by slowing gastric emptying, which is how it helps people feel full longer. But for some users, this slowing becomes permanent damage. The stomach essentially becomes paralyzed.

Symptoms of gastroparesis include severe nausea that never goes away, vomiting undigested food hours after eating, feeling full after just a few bites, painful bloating and abdominal cramping, acid reflux and heartburn, and significant weight loss from an inability to keep food down.

What makes this injury especially devastating is that many plaintiffs report their symptoms continued even after they stopped taking Mounjaro. Some have been diagnosed with permanent gastroparesis that will require lifelong treatment.

2. Acute Pancreatitis

Pancreatitis is severe inflammation of the pancreas, and it can be life-threatening. The pancreas plays a critical role in digestion and blood sugar regulation. When it becomes inflamed, the consequences can be serious.

Reports of pancreatitis linked to Mounjaro have surged. According to the UK health agency, 181 Mounjaro users reported acute pancreatitis as a side effect, with 101 of those reports coming in 2025 alone. Many patients required hospitalization.

Symptoms of acute pancreatitis include intense upper abdominal pain that radiates to the back, nausea and vomiting, fever, rapid pulse, and tenderness when touching the abdomen.

In December 2025, the FDA required Eli Lilly to strengthen Mounjaro’s label to emphasize acute pancreatitis risks, an acknowledgment that this danger was not adequately communicated before.

3. Vision Loss (NAION)

Perhaps the most frightening injury linked to Mounjaro is sudden vision loss caused by a condition called NAION, or non-arteritic anterior ischemic optic neuropathy. This is essentially a stroke affecting the optic nerve, and it can cause permanent blindness.

Recent research has indicated a possible link between GLP-1 drugs like Mounjaro and an increased risk of NAION, particularly in patients with diabetes. The vision loss can occur suddenly and without warning.

These claims have become so numerous that a separate federal lawsuit, MDL 3163, has been established specifically to handle vision loss cases related to GLP-1 medications. Mounjaro does not currently list NAION as a potential side effect on its label despite mounting evidence of a connection.

The core argument in Mounjaro lawsuits is that Eli Lilly knew or should have known about these serious risks but failed to adequately warn patients and doctors. Plaintiffs argue they never would have taken the drug if they had understood the true dangers.
